We feature an accredited Chest Pain Center, certified Stroke Center, dedicated Heart Center, a state-of-the-art Spine Clinic, and Memorial\u2019s maternity center, Special Beginnings. The Pavilion at Memorial Health is our behavioral health center in Jacksonville, Florida. This special facility is designed to help individuals in need of mental health treatments who may be experiencing depression, PTSD or other mental illnesses. Our behavioral health specialists understand the challenges people face when dealing with mental health conditions. Each of our psychiatrists, therapists, psychologists and counselors provide crucial care in a safe and supportive environment. The primary goal is to provide behavioral health services that create stability and self-sufficiency for each of our patients. We treat each of our patients as individuals and always seek to provide them with the support and comfort they deserve. The Social Worker plans, organizes, directs, defines and coordinates clinical services necessary to meet the legal, organizational, medical staff and regulatory agencies guidelines. Provide case management, education, group and individual therapy, and discharge planning. Performs community and educational activities. Licensure: License REQUIRED, eligible to practice in state. Education: Masters Degree in Social Work Experience: 2-4 years in social services and or case management preferred.Demonstrates working knowledge of acute behavioral health criteria, level of care requirements and medical necessity. Basic Computer knowledge including Outlook, Excel, Power Point and word required.
