Role Summary:

The RegisteredNurse (RN) Health Advisor is responsible for providing personalizedcoaching, medical decision support, and information on potential gaps in careto customers through telephonic or face to face assessment, education, supportand referrals to various health and wellness programs. The RN willuse techniques such as motivational interviewing, behavioral change and shareddecision making to help people achieve optimal health and well-being.

results:

vProvides 1:1 telephonic or face to face coaching to members around their behavior change to improve health status, reduce health risks and improve quality of life.

vAssesses readiness to change and implements actions to assist customers in moving through stages of change to reach their personalized goals.

vEmpowers customers to formulate communication plan for provider interactions.

vProvide decision support where multiple treatment options are available*Utilizes health prevention guidelines and general health and wellness strategies to helps customers improve health related outcomes.

vWork with members on opportunities to close gaps in their overall health status

vDocuments interactions and interventions per program requirements. Educates and refers customer on available health resources and Cigna Medical Management and Cigna Behavioral Health programs as appropriate

vDrive efficiency and outcomes (including but not limited to engagement, retention, avoided readmissions, goal progress/achievement) through pre post process including queue management, call and assessment process

vConsults with and assists team members including health educators, pharmacist, behavioral clinicians and medical directors as appropriate.

vProvides suggestions for improvements and enhancements to wellness coaching programs based on data and experience

Performother related duties incidental to the work described herein.

Qualifications:

Minimum

m

ents:

vActive unrestricted Registered Nurse (RN) license in state or territory of residence of the United States

vEligibility to obtain additional state licensure based on business need as required by Cigna.

vRN with current licensure, Bachelor’s degree preferred

vA minimum of three (3) years of clinical experience

vDiverse Clinical experience preferred

vExperience in the use of applied behavior change theories such as cognitive behavior therapy and motivational interviewing.

vDemonstrated success in working effectively with individuals with chronic conditions.

vWorking knowledge of evidence based medicine standards

vStrong computer skills and experience with Microsoft Office software.

vExcellent interpersonal skills and the ability to work in a team environment.

vDemonstrated ability to set and follow through on identified priorities.

vExcellent written and oral communication skills are essential.
