If you are self-confident, enjoy exploring new opportunities, , it’s time to join our dynamic healthcare team!  The Medical Records Coordinator is responsible for the completion of clerical and administrative Work Flow tasks to assist the Administrators/Clinical Team Leaders in improving the clinical and financial performance of the agencies.

Key responsibilities include:

Maintaining patient medical records in our electronic medical records systems

Processing orders to the appropriate physician for signature including but not limited  to  Plan of Care/485,  Verbal  Orders,  Add-on  Evaluations,  Discharges,  Follow-ups, Resumption of Cares and  Transfers to Inpatient Facilities.

Participates in the processes of the agency, seeking to identify opportunities for performance improvement.
Reports significant patient findings to the Administrator  and collaborates on actions or interventions to be taken.
Works to maintain positive, collaborative relationships with patients, caregivers, referral sources, and/or physicians.
Participates in internal staff meetings and training activities, as assigned.

Coordinates with branch staff as needed. 

Various clerical duties as assigned

Job Requirements
Education

High School Diploma or Graduate Equivalent Degree. (G.E.D.)

 

Experience

Minimum of three (1) years of active experience in the health care setting, preferred

Working knowledge of medical terminology and Medicare/Medicaid rules preferred.

 

Skills & Abilities

Possesses excellent customer service skills.

Has a positive attitude and willingness to learn throughout employment.

Self-starter with excellent verbal and written communication skills. Ability to work with both clinical and non-clinical staff members.

Willingness to learn and utilize software solution, inclusive of Divisional Work Flow Tasks.

 Must be organized and have the ability to manage multiple tasks & priorities at any given time.

 Willingness to learn home health regulatory standards related to job-specific duties, including but not limited OASIS, Condition of Participation, state-specific licensing rules, diagnosis coding, and internal policies.
