Riverside Community Hospital, Founded in 1901, is a 478 licensed bed, full-service acute care hospital in the heart of the Inland Empire. RCH houses the largest Emergency Room and Trauma Center in the Inland Empire at 50 beds with an average wait time of only 15 minutes. Centers of Excellence include the HeartCare Institute, offering invasive and non-invasive cardiac procedures, Center of Excellence for Surgical Weight Loss, the Transplant Program and a Level II Neonatal Intensive Care Unit.

POSITION SUMMARY

The Case Management Coordinator provides concurrent review of patient care for the purpose of resource monitoring, allocation and coordination of continuing care. The coordinator supports the case management and hospital policies and procedures.

Reports To: Director, Case Management

POSITION REQUIREMENTS

LICENSURE/ CERTIFICATION/REGISTRATION

* Current California RN License.

EDUCATION:

* Bachelor s Degree preferred

EXPERIENCE:

* Minimum of 5 years recent patient care experience is preferred.

* One year acute hospital case management preferred.

SPECIAL QUALIFICATIONS:

* Able to demonstrate flexibility and adaptability.

* Able to demonstrate excellent verbal and written communication skills.

* Working knowledge of current health care issues.

* Demonstrate ability to perform with minimal supervision.

* General office, filing and clerical abilities to maintain accurate records
