\u003ch5\u003e\u0026bull; Partners with provider on chronic care management and complex patient management.\u003c/h5\u003e
\u003ch5\u003e\u0026bull; Performs medical flip visits with provider for hospital discharges, ER follow-ups, and other complicated visits.\u003c/h5\u003e
\u003ch5\u003e\u0026bull; Facilitates general day-to-day flow of the clinic in close collaboration with other team members.\u003c/h5\u003e
\u003ch5\u003e\u0026bull; Serves as liaison between outside providers/health care entities and the clinic.\u003c/h5\u003e
\u003ch5\u003e\u0026bull; Collaborates with appropriate teams to design and test workflows and RN integration into the care team.\u003c/h5\u003e
\u003ch5\u003e\u0026bull; Performs patient, family, and caregiver education including motivational interviewing and patient centered health action plans.\u003c/h5\u003e
\u003ch5\u003e\u0026bull; Assists in improving the performance of identified quality measures and population health initiatives.\u003c/h5\u003e
\u003ch5\u003e\u0026bull; Triages patients in clinic and over the phone as requested.\u0026nbsp;\u003c/h5\u003e

\u003ch5\u003e\u003cspan style=\u0022text-decoration: underline;\u0022\u003eEducation/Experience:\u003c/span\u003e Associate degree in nursing required. Valid RN license in the state of Washington or Idaho required upon hire. CPR/AED certified for a Health Care Provider required. Nursing experience, preferably within a community health setting required. Case management experience preferred. Experience with electronic health records preferred.\u0026nbsp; Valid driver\u0026rsquo;s license and insurance required.\u0026nbsp;\u003c/h5\u003e
\u003ch5\u003e\u003cspan style=\u0022text-decoration: underline;\u0022\u003eSkills:\u003c/span\u003e Excellent customer service, oral and written communications skills required. Compassion for patients required. Must be able to work well in team environment. Ability to multi-task in fast-paced work environment, with ability to anticipate future needs required. They are required to use hands to finger, handle or feel over two-thirds of the time, as well as reach with hands and arms. Climbing or balancing occurs less than one-third of the time; while stooping, kneeling or crouching occurs between one third and two-thirds of the day. Communicating by talking / hearing occurs over two-thirds of the day. Amount of time spent lifting or exerting force is about 50% for up to 10 pounds and less than one-third of the time up to 25 pounds. Rarely is there a need to lift more than 26 pounds.\u003c/h5\u003e
