Utilizes case management for patient support, assistance with safety planning, and other supportive activities such as contacting ambulance services, seeking collateral sources of information, etc. Collaborates with and utilizes information from other treatment team members. Seeks consultation from supervisor and prescriber and/or on-call psychiatrist as needed to promote patient care and to problem solve.\u003c/li\u003e
\u003cli\u003eKeeps all parties informed throughout the process including delays regarding hospitalization and/or ambulance transport. Ensures disposition is communicated to referral sources and other members of the treatment team. Relays critical information to treatment team members to promote continuity of care between daytime and afterhours staff. Communications are provided by therapist or therapist ensures hotline or case management staff are communicating on behalf of therapist.\u003c/li\u003e
\u003cli\u003eUnderstands the role of Patient Care Standards (PCS) as a measure of our commitment to serving those in need and ensuring that access to care is provided. Ensures PCS standards are met and actively utilizes and responds to strategies provided through supervision when there are challenges in meeting these standards.\u0026nbsp;\u003c/li\u003e
\u003cli\u003ePerforms tasks necessary to support revenue including timely and accurate documentation within departmental guidelines; obtaining accurate patient demographics and insurance information; and prior-authorizations for services when applicable.\u003c/li\u003e
\u003cli\u003eIs able to effectively and efficiently utilize EMR and video-conferencing software. Prioritizes the use of video-conferencing when providing tele-crisis services. Utilizes work email on a daily basis keeping current with FRMC and departmental communications. For new hires, these skills are in place by the end of the probationary period. Ensures all electronic forms of communication, that include PHI, are compliant with HIPAA and 42 CFR laws regarding confidentiality.\u003c/li\u003e
\u003cli\u003eProvides and documents other outpatient behavioral health services as needed and within departmental guidelines.\u003c/li\u003e
